Enrolments can now be fully completed online.

Below there are three options for enrolling with us. Choose the one that suits you best.

 

Option 1 : Register Online

  • Click on the Start Enrolment link to complete the enrolment forms online and upload a photo or a scan of your Passport, NZ Birth Certificate or Community Services Card +/- copies of visa or permits if applicable.
  • Once you have completed your online form, you will receive an email confirming we have received it.
  • The enrolment team will send you a portal enrolment invitation as confirmation of your enrolment. Our portal is called MyIndici and once registered it is available via Desktop & App (Android & IOS). If you decline the portal, you will receive an email or text message to confirm your enrolment.
  • This will usually be within 7 to 10 days.
  • If further information is required to complete the enrolment, our team will be in contact via email to request this.

Option 2 : Download and fill out enrolment forms

Click the link to download the enrolment forms, then print them out, fill them in and then return to us by email, post or drop them in.

Option 3 : Come to the clinic and fill out the forms

 

The NZ Health system requires us to have proof of your eligibility to receive subsidised services and to enable referral for laboratory tests and to hospital services.
So we will need to have photos or scans of your Passport, NZ Birth Certificate or Community Services Card.
Australian citizens who have been or are intending to stay in NZ for at least 2 consecutive years are eligible to enrol.
Also if you have a work visa or permit that shows you are able to be in NZ for at least 2 years, or if you are a refugee, you will be eligible but we will require photos or scans of the documentation.
